Welcome to Skyview Lacrosse Player Registration

Please note:  Current US Lacrosse membership is required for all Skyview Lacrosse players.  US Lacrosse membership provides essential secondary insurance coverage for players as well as providing numerous educational benefits.  US Lacrosse membership must be completed directly with US Lacrosse before the start of practices and your player's membership number and expiration date must be on the registration paperwork.  US Lacrosse membership dues are in addition to Skyview Lacrosse registration fees and can be completed directly with US Lacrosse
 
Refund Policy: Registration fees are non-refundable. Skyview Lacrosse's refund policy is based upon our obligations for field reservations, coaching stipends, equipment purchases and administrative costs such as insurance which are based on registration numbers. Partial refunds for Spring season may be granted on a case by case basis in the event of injuries or other situations beyond a player's control.

Registration fees pay for field rentals, coaching, referee fees, club memberships and insurance, field equipment, and other associated club costs. New players will receive a helmet wrap, shooting shirt, team practice pinnie and shorts with their paid registration.
 
Cash and Checks will be accepted.  We will be able to accept credit card payments using Square.  The transaction fee for Square charges are 2.75% of the transaction total.  The purchaser will be responsible for this transaction fee to help alleviate unnecessary costs to the program.  We appreciate your understanding!
 

Completed registration, the Skyview Lacrosse Athletic Participation Packet 2016-2017 and a copy of the player's sports physical will be required.  Skyview Lacrosse Athletic Participation Packet 2016-2017 is available on the website under the “Documents” tab.

Sports physicals are good for two years from the date of the physical.  If there is more than one page, the athlete’s name must be on both pages.  The physical must be dated no earlier than June 2015.

Skyview Lacrosse is committed to making sure that every athlete that wants to play lacrosse has an opportunity to do so, no player will be cut from the program.
